Position Description
Veridia Health is seeking a full-time Patient Care Coordinator to serve as the primary link between patients, clinicians, and the broader healthcare system. This role focuses on coordinating care plans, managing patient communications, maintaining medical records, and ensuring a seamless patient experience. The ideal candidate is highly organized, service-oriented, and passionate about delivering exceptional patient care within a personalized, preventive healthcare model.
Responsibilities include:
- Participating in patient appointments and documenting care plans
- Coordinating appointments, preventive screenings, and diagnostic testing
- Maintaining accurate medical records and EMR documentation
- Managing medication and supplement records
- Communicating with internal and external healthcare providers
- Assisting with insurance authorizations and appeals
- Onboarding new patients and serving as an ongoing point of contact
- Responding to patient inquiries and supporting care plan follow-through
- Tracking patient tasks and follow-ups using project management systems
- Identifying process improvements and supporting operational efficiency
- Collaborating with the clinical team on patient care planning and engagement strategies
Position Requirements
- Bachelor’s degree
- Strong interest in medicine and patient care
- Exceptional organizational skills and attention to detail
- Excellent written and verbal communication skills
- Ability to build trusted relationships while maintaining confidentiality
- Proven ability to work effectively in a fast-paced team environment
- Familiarity with EMR systems and healthcare technology preferred
- Understanding of U.S. healthcare operations and regulations preferred
- Strong problem-solving skills and ability to leverage digital tools efficiently
