STUDENT MEDICAL INSURANCE PLAN 2002-2003

The Student Medical Insurance Plan (Insurance Plan) offers reasonably priced medical benefits that have been designed to supplement on-campus health care. Enrollment in the Insurance Plan provides you with prescriptions, consultations, long-term counseling, x-rays, and outside laboratory tests. The Insurance Plan is administered by Chickering Benefit Planning Insurance Agency, Inc. and is underwritten by Aetna Life Insurance and Annuity Company.

The Student Medical Insurance Plan offers two options for coverage. The Basic level of the Plan is designed to provide a level of coverage that is appropriate for most college students. The Comprehensive level includes all of the benefits provided by the Basic level, as well as more extensive benefits in certain areas. For full-time students, enrollment in the Basic level of the Plan and billing of the premium are processed automatically. Students may choose to enroll in the Comprehensive level of the Plan or may request to waive the Plan by providing proof of a comparable level of coverage. It is not possible for any student to be enrolled in the Insurance Plan without being enrolled in the CMCP.

Students must re-elect (enroll or waive) their chosen plan each Fall. Changing plans mid-year is not permitted.

Special Category Students: Matriculation & Facilities, Extended Residence, Defense Fee, MFA Research, and Designated Executive Programs

Students in the above categories may waive the Basic level of the Student Medical Insurance Plan by completing a waiver form, which will remove the Health Service Fee charges as well. If, after waiving these charges, a student wishes to continue to be eligible for care in the CMCP, the student must come to the Health Services at Columbia Insurance Office in Room 401 of John Jay Hall to complete an Enrollment Form, or call (212) 854-3286. Be prepared to submit proof of alternate medical insurance coverage. Students may also receive care in Primary Care on a fee-for-service basis ($50 per visit).

Part-Time Students
Part-time students may enroll in the Basic or Comprehensive levels of the Student Medical Insurance Plans by completing an insurance enrollment form. Part-time students must also pay the Health Service Fee in order to enroll in the Insurance Plan. The appropriate charges will be added to the Student Account Statement.

Dependent Coverage
Eligible students who enroll in the Insurance Plan may also enroll their eligible dependents in the Basic level of the Plan.
